9 legionella assesments were overdue in North at the end of the Q3 reporting period. Although work order instructions were issued to the Contractor well in advance, an unusual combination of staff sickness, planned leave, and urgent unplanned absence meant that key personnel were not available to identify that the Contractor had not attended these sites as scheduled. Once this issue was identified, appointments were promptly arranged for all outstanding assessments.
